The Worcester Health Center is included in a list of once-troubled nursing homes that have improved patient safety, according to a new report by the Centers for Medicare & Medicaid Services.
The nursing home was most recently inspected in April, with the federal agency listing it among 88 such centers where quality issues were discovered in past inspections.
An inspection last November found Worcester Health Center allegedly failed to protect each resident from physical, mental or sexual abuse, physical punishment, and neglect. An inspection last September found 19 deficiencies, compared to a national average of less than eight, according to federal documents.
The most recent inspection found just six deficiencies. In one issue soon corrected, inspectors determined the facility failed to ensure it was free from accident hazards and failed to provide adequate supervision to prevent accidents.
A Worcester Health Center manager declined to comment Friday on the most recent inspection.
Worcester Health Center is one of two Massachusetts facilities on the Centers for Medicare & Medicaid Services' list of troubled programs, which it calls special-focus facilities. Sweet Brook Rehabilitation & Nursing Center in Williamstown was added to the list for the agency's most recent report June 26 after an inspection found 29 health citations.
The Centers for Medicare & Medicaid Services inspects such facilities twice as often as other nursing homes. Facilities not improving can be removed from the Medicare and Medicaid programs.
Two site reviews showing improvement can result in a facility being removed from the federal troubled-facility list.
Worcester Health Center, formerly the Autumn Village Skilled Nursing and Rehabilitation Center, is located at 25 Oriol Drive off Lincoln Street.
