The University of Massachusetts Medical School has been awarded a contract to support a patient-centered medical home initiative with the Rhode Island Chronic Care Sustainability Project.
In a patient-centered medical home, a primary care provider and team coordinate a patient’s health needs, managing visits to specialists and hospitals, giving patients reminders for check-ups and tests and making sure chronic conditions are being managed appropriately.
UMMS will organize meetings among project stakeholders, manage budgeting and other administrative tasks, and oversee its strategies for meeting key objectives.
