Seven Hills hires long-term care director

Worcester human services nonprofit Seven Hills Foundation has hired longtime nurse practitioner Christine Lens as director of the Massachusetts Care Coordination Network, responsible for care coordination of 4,500 patients with long-term care needs, according to a statement issued Tuesday.

Lens, a registered nurse who holds an MBA, was most recently a regional clinical care manager at the New England Quality Care Alliance, the nonprofit physician network owned by Boston-based Tufts Medical Center. There, Lens was the liaison for the Tufts Medicare Preferred Program and contributed to improved performance for the the network, as well as the implementation of complex care management for several of the local care organizations within the network, Seven Hills said.

Lens has 35 years of experience working as a nurse, including 25 years as a nurse practitioner, according to Seven Hills.

The MCCN is a long-term services and supports community partner under the state’s new MassHealth Accountable Care Organization program, providing care coordination for patients with long-term care needs in the Northern, Central and Southern regions of Massachusetts beginning June 1.

Seven Hills Family Services, an affiliate of the Seven Hills Foundation, is the lead agency in this partnership of five health and human service agencies in Central and Eastern Massachusetts.
