MeHI announces grant program to improve electronic records sharing

The Massachusetts e-Health Institute (MeHI), a division of the state-backed Massachusetts Technology Collaborative, has launched a new grant program to help health care agencies improve the exchange of electronic health records with other providers.

The Connected Communities Implementation Grant Program is soliciting proposals from groups working together to use information technology to improve health care coordination in the communities they serve.

The program is designed to increase the electronic exchange of health information and strengthen workflows to help providers better address transitions of care and care management needs within their communities as a way to improve outcomes and cut costs, according to a statement from MeHI, which provides subsidized support of health care IT initiatives among Bay State providers.

Laurance Stuntz, director of MeHI, said the program “provides us with an opportunity to support impactful health IT programs driven by the priorities in individual communities.”

“Our hope is to receive proposals that identify region-specific roadblocks to sharing information, engage a broad cross-section of healthcare stakeholders, and address the unique needs of patients in that community through the use of technology,” Stuntz said in a statement.

Health care providers and insurance plans that are involved in treatment, payment and health care operations are eligible to apply. Projects that receive funding must seek to:

-Strengthen connections between providers in a defined geographic region;

-Improve provider access to clinical information, such as discharge plans and test results as patients transition between care settings; and

-Support improved outcomes and efficiency in treatment, including measureable reductions in hospital readmissions, medical errors, and duplicative tests, through improved cross-setting care management.

Awards will be made in a phased process. MeHI will initially award successful grantees a planning payment of up to $25,000, which participants will use to develop key project requirements. Payments will be made when milestones are achieved. Depending on individual projects, up to a maximum grant of $500,000 per project.
