The Massachusetts Health Policy Commission (HPC) is offering $10 million in grant funding to applicants with proposals to shift health care away from high-cost providers and toward less expensive, community-based settings, according to a statement from the agency Tuesday.
The program, known as The SHIFT-Care Challenge, emphasizes partnerships between community organizations, accountable care organizations (ACOs), and hospitals and health systems. It’s designed to foster care delivery models moving care for patients with complex medical needs away from more expense, acute-care hospitals and toward community-based providers.
The HPC issued a request for proposals for the funding round, with a deadline for proposals on April 6. Massachusetts providers and provider organizations are eligible to apply for up to $750,000 in funding.
“The HPC will fund the most competitive innovative care delivery models that have the potential to offer new lessons as we press forward in our efforts to improve health while working to contain health care costs,” HPC Executive Director David Seltz said in a statement.
