Harrington gets $440K grant for chronic disease management

Harrington HealthCare Systems in Southbridge has been granted $440,000 from the state Department of Health and Human Services to help better manage chronic disease.
The funding, from the HHS delivery system reform incentive payment program, will go toward Harrington’s plans to focus on a multidisciplinary approach to chronic disease management beginning with chronic obstructive pulmonary disease and pneumonia.
That approach includes the hiring of care navigators and behavioral health specialists to oversee a patients’ continuum of treatment, including primary care, acute care, behavioral care and post-acute settings, Harrington said in a press release.
“The grant will allow for a nurse to assist in the clinical needs of the disease and a behavioral health specialist to assist in barriers to leading a healthy life,” said Jessica Calcidise, Harrington vice president of nursing and ancillary operations. “Social determinants can prohibit a patient from leading a healthy life with a chronic disease. Behavior modification can be a key to success.”
Harrington has previously been able to expand its patient navigator program through state funding and achieved a 34-percent reduction in the number of emergency department visits for patients with multiple behavioral health issues.
Now, the healthcare system has nearly $1 million in public funding to continue to improve those patient navigator services.
