The Greater Worcester Community Foundation on Monday announced it is looking for grant applicants seeking to fund projects addressing the social determinants to health.
The money is available through the Fallon/OrNda Community Health Fund, which awards approximately $200,000 annually to projects focused on outreach to vulnerable populations; removing barriers to healthcare services; access to coordinated healthcare services; and promoting disease prevention and health promotion programming.
Applications for grants between $5,000 and $25,000 will be considered, with priority given to collaborative proposals. More details are available here.
