After being delayed several months due to discussions over rates, a pilot health insurance program for patients eligible for Medicare and MassHealth benefits will begin offering coverage Oct. 1.
Fallon Total CareSM, a subsidiary of Fallon Community Health Plan, said it will be the first in the nation to launch a “dual-eligible” plan sponsored by the Centers for Medicare & Medicaid Services and the state’s Executive Office of Health and Human Services to address complex medical and mental health needs.
The plan is available to eligible adults in Worcester, Hampden and Hampshire counties. Fallon Total Care navigators will work with members, their caregivers and their health care providers to coordinate services, and assist members with physical, behavioral and social needs, FTC said.
Fallon Total Care is open to people who:
• Are 21 to 64 years old (People who turn 65 after enrollment can continue to be members or select other options.);
• Have Medicare Parts A and B and are eligible for Part D;
• Have MassHealth Standard or MassHealth Common Health as an insurance provider;
• Live in the Fallon Total Care service area; and
• Have no other private insurance.
