Fallon Health of Worcester has opened its sixth Summit ElderCare services branch.
The new location on the campus of D’Youville Life & Wellness Community in Lowell joins locations in Charlton, Leominster, Springfield and two in Worcester. Together, the branches serve more than 1,000 participants, according to Fallon.
Fallon’s Summit ElderCare is a Program of All-inclusive Care for the Elderly (PACE) and serves area residents age 55 and older who are eligible for a nursing home level of care, yet seek to live as independently as possible in their own homes. The program provides patient-centered, all-inclusive and coordinated health insurance, medical care and non-medical supports in one location.
The newly built 13,000-square-foot Summit ElderCare building in Lowell will be able to accommodate 250 participants and approximately 50 staff members, according to Fallon.
