Westborough-based electronic health records and practice management firm eClinicalWorks announced it has expanded its role with Holyoke Medical Center and will help the hospital achieve its goals as an accountable care organization (ACO).
eClinical will provide the hospital with a care coordination medical record (CCMR). In using the program, Holyoke Medical Center will be able to coordinate care and evaluate population health and quality improvement. That will help the hospital become a nationally recognized "patient-centered medical home (PCMH)." Both the ACO and PCMH models coordinate care between patients and health care providers. The goal is to strengthen the physician-patient relationship, leading to better care and reduced costs over time.
The Western Massachusetts hospital, which reported nearly 242,000 outpatient visits last year, already uses eClinicalWorks' electronic health records (EHR) system.
"Our organization serves one of the poorest populations in the commonwealth," said Carl Cameron, chief information and analytics officer at Holyoke Medical. "Using eClinicalWorks' EHR has already aided our providers in delivering comprehensive care. Extending this technology to include eClinicalWorks' CCMR will enable us to better track patients with chronic conditions as well as conduct analytics, care planning and population health reporting initiatives."
The firm's community analytics, included in CCMR, delivers reporting, alerting and messaging capabilities to help manage patients' health.
