UMass Memorial Health Care of Worcester has received a grant from Harvard Pilgrim Health Care to support the coordination of patient care.
The grant was part of a $1-million annual round of giving from the Wellesley-based insurer.
The funds went to 14 physician groups in Massachusetts, New Hampshire and Maine to support patient-centered medical home initiatives, which are designed to engage patients with their primary care physicians to help coordinate all their care.
